Default Jerking 2002 Tranny Problem

I have a 2002 Escape, and when I put in into Drive it sometimes jerks, not alot, it allso take s a few seconds for the revs to drop after starting which I think might be adding to the problem. it doesn;t jerk in reverse, just drive, and it goes through the rest of the gears OK. it has 110,000kms on it.

Can anyone shed any light on this?
