I replace EGR valve. No luck.

What should I have replaced?

Check the DPFE sensor - I had the same issue with my 97 F150. Look for the part on your van, get the part number and look on ebay, it's a dealer only part and will cost you a pretty penny. I got mine for @ $35.00.
