Default Door Handle Removal and Rear A/C Fan help

Well, I have a 1999 Ford Econoline E150 Triton V8 yadda yadda..

Anyways, The rear door and trunk door handle latches have been broken (they slip, and don't open from the outside) or worn through. I've taken off the cushioning panels on the inside to locate and see what the actual problem is. I've figured all that out, but right now the problem is removing the handle. Just the metal one on the outside. I've taken a look at the 'screws' and they're not really extractable. Anyone have an idea of how to take them out?
The screws have a little black aluminium housing which has a head on the outside, and then a screw without a head is placed in the middle of it, and then the rear end of the housing on the inside is split and folded out in a cross.

Now with the Rear A/C Fan, it seems to be making a clicking, only when the rear AC is on. I presume something is in the fan or whatnot, but am having difficulty locating the actual fan itself (the plastic covering does not reveal much..)

Any help would be much appreciated.
