Removing Door Panels

How do I remove the front door panels to install speakers? I've removed all the screws, the bottom part has come up, but the top part won't budge!

There is usually one screw behind the "sail panel", the small plastic trim that covers the bolts for the outside rear view mirror.

The door panel then slides up and comes off. If it does not slide up, you missed a screw somewhere,
